Fee's

First visit: you will spend one full hour with Dr. Fields. This does not include filling out paperwork or time with a nurse or an assistant.

A complete history and physical is done at that time. Then one area will be treated. If more than one area is treated, the price will be adjusted accordingly.
Each visit thereafter: depending on the area treat the price will vary. See below for prices.

you have a Prolotherapy consultation without treatment, this is usually half an hour or forty minutes and is charged accordingly. All other services are based on the time needed. You may ask the front desk how much time is needed for the service you are requesting.

Hormonal assessment or nutritional counseling initial visit is one hour and is charged as such

Payment Policy

Investing in your health should be very important to you. Since the future of health care is changing, you should be involved with this investment. When you make a financial investment, you measure it by how much it grows each year. Investing in your health pays off in many different ways: wellness, health stability, quality of life, energy, happiness and more.

Only you can decide what is fair and if it worth the investment even if you have to pay
out-of-pocket costs for your health care.

We will make every effort to instill the feeling that your time with us is worth it.

As a Fee for Service Office we request that the fee is paid in full at the time of service.
We accept Visa, MasterCard, checks, or cash for payment of services.
We will provide you with a detailed invoice that they can submit to your insurance company for possible reimbursement. We cannot guarantee insurance reimbursement.

Pacific Prolotherapy and Wellness Center does not subscribe to any insurance networks. Insurance billing can be up to 40 percent of a medical office overhead. It involves a lot of time and effort. We rather put that time and effort into caring for our patients instead of billing insurance companies. It also helps us to keep our prices reasonable.

Remember that health insurance is a contract between you and your insurance carrier.

If so inclined, you can contact your insurance carrier and ask them if they cover an MD office visit (codes: 99203 or 4; 99213 or 4.) They will not tell you how much they cover- only if they cover it or not.Most Insurance companies cover MD's office visits. This is what we bill for.

All bills are issued under Dr Fields' medical license (MD). Unfortunately, there is still no guarantee that insurance companies will reimburse for your treatments even after they have said yes to complete or partial coverage.

If requested, we will file your claim with your insurance company on your behalf for no additional charge.

We cannot bill Medicare or Medi-cal since we are not a provider.
Please do not ask us to attempt this as it is not possible.

Both of them are a lot stricter on what they cover. Please make sure you ask them directly about their coverage. Some Medicare/Medical patients may get coverage from their secondary insurance carriers.
